Abstract

Gastritis is the term used when there is epithelial and endothelial damage in mucosa but there is little or no accompanying inflammation, erosions and sub epithelial hemorrhage are most commonly seen at endoscopy mast common cause of gastric is mucosal damage associated with use of aspirin and other NSAIDs and alcohol and other most common cause is H.pylori infection which produce gastritis manly in the antrum of the stomach mucosa appears redden endoscopically and histologically there is epithelial cell damage from local release of cytokine.
Matrix metalloproteinase play control role in many biological processes such as embryognses, normal tissue remodeling, wound healing, and angiogenesis, and disease, such as atheroma, arthritis, cancer and tissue ulceration.
Degradation and remodling of extra cellular matrix is increasingly implicated in the pathogenesis of many inflammatory disease such as gastritis. MMPs play role in these events. During inflammation their deregulations leads to excess degradation of matrix components and lost of tissue integrity MMPs are relased from almost all connective tissue cells in response to-inflammatory stimuli such as cytokeins.
The aim of this thesis is to investigate the expression of these proteolytic enzymes (metalloproteinases 3&9) in Helicobacter pylori associated gastritis.
. this thesis done upon 30 patients already diagnosed to have H.pylori associated gastritis with or without ulcer disease they were subdivides into three groups
a) Ten Patients who had been treated before by a full course of anti H.pylori regimen.
b) Ten Patients who had been treated previously by frequent courses of H2 Blocker and antacid.
c) Ten Patients received no previous treatment.
and compared with control group ten subject as plasma mean concentration of MMP-3, MMP-9 was measured.
which denoting significant elevation of MMP-3 and MMP-9 in H.pylori (+ve) group than those of control group H.pylori (-ve) group.
So MMP-3 and MMP-9 are increasing according to disease activity so can be used as bio marker of H.pylori associated gastritis than H.pylori (-ve) group.
So we can conclude that MMP-3 and MMP-9 have a role in the pathogenesis of gastritis with H.pylori so can be used as bio marker of gastritis with H.pylori.
