Abstract

lesions frequently preseIll a diagnostic dilemma. The differential diagnosis includes hemangiomas, hepatomas, meta:;lases adenomas. focal fat and others. The aim of 1]1IS work is to study the role of color Doppler sonogr<1phy in <1ssessment of the \’asclllarity of focal hepatic lesions and determine ItS liability to add diagnostic data regarding the lesion characterization.
This study \as conducted on 65 patients with focal hepatic lesions selected on basis of llltrasonographic examination.
They were divided into 3 groups according to histopathological examination
Group I: 27 cases proved to be HCCs.
G rou p 11: 18 cases proved to be metastatic deposits. Group Ill: 20 cases proved to be benign lesions.
Our patients were subjected to:
SUMMARY & CONCLUSION Metastases showed detour pattern in 3 (16.7%) aut of 18 cases.
Mean peak systolic velocity was 27 cm /sec.
No significant difference in mean peak systolic \elocity and mean resistive inde.\ for HCCs and metastases.
• Col or Doppler sonography is useful in evaluating the \’ascularity of hepatic rumors.
• Calor Doppler sonography is non-InvaSIve method that can alleviate the need for biopsy in considerable percent of patients.
• In some few cases the pattern of flow can help in differentiation of HCCs from metastatic deposits However, biopsy shoul~ be included 111 the diagnostic work up of such lesions for more .Further studies are needed to widen the spectrum of informations gained by color Doppler sonography and to employ it 111 new goals like grading of hepatic tumors and confirming treatment effectiveness following therapeutic procedures such as percutaneous ethanol injection therapy and radiofrequency ablation therapy.
