Abstract

Conservation laryngeal surgery refers to techniques that preserve
the sphincteric, respiratory, and phonatory functions of the larynx. The
techniques vary from simple excisions of small laryngeal cancers to
technically difficult resections of the supraglottic region and hypopharynx
and reconstruction of the remaining laryngeal structures. All of these
conservative laryngeal techniques are based on the knowledge of the exact
extent of the tumor and its pathophysiologic course in terms of local tumor
spread and metastatic pathways.
Conservation laryngeal surgery includes two main categories,
vertical partial laryngectomies, which varies from simple cordectomy to more
sophisticated surgical techniques as hemilaryngectomy, anterior frontal
laryngectomy and frontolateral laryngectomy. The other category is
hO,rizontal partial laryngectomy, which includes supraglottic laryngectomy
and supracricoid .laryngectomy, In the last decade of the 20th century,
endoscopic or transoral partial laryngectomies, using C02 laser, have been
introduced.
