Abstract

The aim of the work is to evaluate the fetal condition and to determine the best predictor of adverse outcome in post date pregnancies among different fetal surveillance tests BPP, NST, CST, AFI and middle cerebral to umbilical artery Doppler ratio.
On conclusion:
1- Postdate pregnancy is associated with increased incidence in intrapartum fetal distress especially in the presence of oligohydramnios and meconium.
2- The pest specificity and positive predictive value can be achieved by the combined testing parameters (BPP < 4 + C/U ratio < 1, 05).
3- Middle cerebral artery resistance index is a good predictor of fetal oxygenation status at birth and should be used to identify the post date pregnancy at risk for morbidity and mortality.
4- Umbilical Doppler velocemitry is un likely to be useful for routine assessment of fetal status in post date pregnancies due to increased placental resistance as reflected, by abnormal US velocity wave forms does not seem to play a role in the pathology associated with postdate pregnancy.
