Abstract

To compare between two different glaucoma surgery techniques : microtrabeculectomy and conventional trabeculectomy as regard IOP control and postoperative complications.
Subscleral trabeculectomy has been subjected through the years tonumerous modifications regarding the type of the conjunctival flap, theshape of the scleral flap, use of antimetabolites and the type of the suturematerial.
It has been a definite tendency over the past few years to minimizeusage of suture material up to advocating sutureless surgery and this isapplicable to cataract and glaucoma surgery.
These trends have been supported by the evident greater woundstability, less tissue reaction, less tendency to postoperative astigmaticshift and hence rapid postoperative physical and visual rehabilitation.
The present study was conducted to compare between theconventional subscleral trabeculectomy and the microtrabeculectomyregarding the IOP control and the postoperative complications.
This study was carried out on sixty eyes of sixty patients divided totwo groups each group contains thirty eyes. Conventional trabeculectomywas done to the patients in the first group,and microtrabeculectomy wasdone to the patients in the second group according to Ophir 1999. Inmicrotrabeculectomy a 3mm conjunctival flap is performed, no orminimal cautery is used, then a scleral tunnel is performed using acrescent knife, followed by opening the anterior chamber andtrabeculectomy is done by the Kelley Descemet’s membrane punch,iridectomy is done and finally the conjunctival flap is closed with 10-0nylon sutures.
