Abstract

The aim of the work is to:
1) Study the HCV genotypes present in EL-Minia province.
2) Find out the correlation between HCV genotypes and the clinical, laboratory, and liver histopathological findings of the patients.
from the study we conclude that:
1- The most common HCV genotype present in our locality is genotype 1V (72%), followed by genotype 111 (25.3%), genotype 1 (16%) and genotype 11 (19.3%).
2- No correlation between the HCV genotypes and age, sex, residence or risk factors of patients with chronic HCV infection.
3- No significant difference between the HCV genotypes present and the mean serum levels of AST and/ or ALT.
4- No significant correlation between HCV genotype present and the liver histopathology, except that there was high significant correlation between presence of mixed genotypes (1& 11 and 111 & 1V) and hepatucellular carcinoma (P= 0.0001).
5- As regard the risk factors of HCV infection, we found that 56 cases (74.7%) out of the 75 included patients had positive history of risk factor for HCV infection, while 19 cases (25.3%) were with unidentifiable risk factor for HCV infection.
6- The most common symptom of the included patients were easy fatigability, anorexia, abdominal pain, abdominal distension, weight loss, epistaxis, hematemesis and melena.
7- The most common signs in the included patients were fever, palmer erythema, shrunken liver and edema of the lower limbs. Also, muscle wasting, jaundice, spider naevi, gynaecomastia were significantly common signs especially in group 3.
8- Ten cases (13.3%) out of the 75 included patients were diabetic (blood glucose level more than 200 mg/dl): 8 cases of them in group 3c and 2 cases in group 2.
9- No significant correlation between the liver histopathological findings and the mean serum ALT & ST levels (P= 0.517).
10- Ten cases (13.3%) out of the 75 HCV RNA-positive patients were also HBsAg positive. It was found that HCV/ HBV co-infection was significantly associated with hepatic malignancy (P value = 0.01).
