Abstract

Obesity is becoming a serious problem because its prevalence is increasing and it is well known now that, there is a significant negative correlation between lung mechanics and obesity The severely obese patients have varying degrees of intrinsic reduction of expiratory flow rates and lung volumes. They are predisposed to postoperative atelectasis, ineffective clearing of respiratory secretions, and other pulmonary complications. Abdominal surgeries lead to severe pain that is associated with changes in respiratory mechanics, shallow breathing and impaired ability to cough. Epidural anesthesia for perioperative analgesia would reduce the magnitude of postoperative deterioration in lung function in obese more than in non obese patients compared with systemic opioid We designed this study to compare between the effect of thoracic epidural analgesia and systemic opioids to determine which route of them that would offer any advantage on perioperative lung mechanics on obese patients undergoing major gynecological procedures We have studied Ninety patients scheduled for major gynecological procedures. They were randomly allocated into 2 equal groups:- Group I :- were assigned to receive general anesthesia in combination with intravenous opioids Group II :- were assigned to receive general anesthesia in combination with epidural anesthesia.
