Endothelial cells produce the 21 aminoacid peptide endothelin-
I, which is formed from its big precursor, big endothelin, via the
activity of converting enzyme. The basal production of the peptide is
stimulated by epinephrine, angiotenisn Il, aginine vasopressin,
transforming growth factor B, thrombin and hypoxia. ET-I is mainly
produced by vascular endothelial cells. In vascular smooth muscle,
endothelin binds to a specific receptor (ETA, subtype), resulting in
an increase in intracellular calcium and vasoconstriction. (Luscher,
1992).
Coronary arteries have a high density of binding sites for
endothelin-l and endothelin -2 and infusion of these peptides causes
intense coronary vasoconstriction (Chester et al., 1989).
In this work we studied the changes which occur in plasma
endothelin in patients with acute ischemic syndrome. And a trial to
find out the relation between plasma endothelin level and left
ventricular wall motion score index, cardiac enzymes.
S .. ixity individuals were included in this study comprising 20
patients with unstable angina, 20 patients with acute myocardial
infarction and 20 age and sex-matched healthy control.
