Abstract

Dyspepsia represents one of the important gastrointestinal disorders because of its remarkably high prevalence in general population and its impact on quality of life.
One of the challenges for the primary care physician is to determine optimal treatment line for the patients presented with new onset or previously known uninvestigated dyspeptic symptoms, so the clinician must decide which investigations are needed and optimal drug therapy (selection of appropriate agent, duration of treatment) and appropriate follow up plan.

Due to its dependence on both subjective determinants and diverse country-specific circumstances, the definition and management strategies of dyspepsia are still variably stated.
Many international guidelines were approved to approach the functional dyspepsia; these guidelines assess the risk factors and environmental related specific factors, considering some of these factors important while the other factors less important.
Risk factors for functional dyspepsia had been shown to include gender, helicobacter pylori infection and underlying psychological disturbances whilst environmental / life style habits such as poor socioeconomic level, smoking and ingestion was regarded either has minor effect or had no effect at all .
In present work we study the impact of these risk and environmental related factors in addition to other factors that are related to our locality such as Bilharziasis, chronic compensated liver disease and parasitic infestation on the approved guidelines for the management of dyspepsia .

A total of 100 subjects were included in the study and were assigned to the following 3 groups:
Patients with reflux like symptoms(20 cases), Patients with ulcer like symptoms (30 cases), Patients with dysmotility like symptoms (50 cases) .
All patients were subjected to the following:
- Full history taking with stress on symptoms suggesting dyspepsia and complete clinical examination.
- Routine investigations: stool analysis, rectal snip, Random blood sugar, complete blood picture, Liver function tests, Kidney function tests and body mass index.
- Standard 12 leads electrocardiography.

-Abdominal ultrasonography to assess the liver, spleen, gall bladder and portal vein state.

- Special investigations in form of: - Upper G.I Endoscopy, Detection of H.Pylori and Monitoring of gastric motility and evacuation using real time ultrasonography.
From the results of the previous investigations we can concluded that the community related environmental and the risk factors have an important impact on the internationally guidelines for the management of dyspepsia in our locality.
