Abstract

The objective in the surgical treatment of Legg-Calve-Perthes’disease is to achieve containment and thus minimize the development of deformities and subsequent osteoarthritis.
Favorable results have been achieved when the involved femoral head has been contained during the active phase of the disease .
The advantage of operative containment is that the child can resume a normal life without further interruptions for treatment.
In our study we treated 30 patients suffering from Perthes’disease surgically by proximal femoral varus osteotomy, 23 patients were male, and 7 females, 16 patients are affected in the right side and 14 patients are left side, history of transient synovitis was positive in 15 patients.
Preoperative clinical assessment using the criteria of Harris hip score used and patients classified into 15 poor group, 8 fair, and 7 good.
13 patients classified as Herring B and 17 patients classified as Herring C.
All these patients are treated surgically by proximal femoral varus osteotomy, with average period of follow-up of about 24 months.
We achieved a high clinical success rate as,the poor group was in 50% (15) of patients’ preoperative, while 6.7%(2) post-operative, the fair group was in 23.3% (7) of patients’ preoperative, while 6.7% (2) post-operative, was 26.7%(8) preoperative, while 20% (6) post-operative, was in 0% (no patients) preoperative, while in 66.7% (20 patients) in post operative group.
The high success rate achieved in male and younger patients, and Herring B.
In additionthe goal of treatment for early return to functions, including sports, pain and freedom from deformity achieved.
