Abstract

Urethral substitution with BM graft and penile skin flaps provided appropriate management for penile and lengthy stricture involving the anterior urethra. The results of BM substitution urethroplasty ware comparable to penile skin flaps. Buccal mucosa was additionally dictated by the presence of deficient penile fascia and local skin, and also the presence of BXO as an etiological factor for urethral stricture. The successful use of TEBM mucosa for substitution urethroplasty gave new hope for engineered material that could replace the native buccal mucosa, however the TEBM exhibited abnormal exudates and fibrosis in 2 out of 5 cases. This could be attributed to its use or might be due to BXO presentfrom the start. The use of TEBM led to investigation of new biodegradable material to replace the allogeneic dermis, act as a scaffold for growth and proliferation of the patient oral cells. The biodegradable scaffold was successfully sterilised with gamma irradiation and peracetic acid, the cells were able to attach and proliferate. The engineered constructs had ultimate tensile stress comparable to native buccal mucosa but were unable to match its elasticity modulus. The experiments provided better understanding of the nature of the biodegradable PLGA scaffold and showed that, during every step throughout processing, sterilisation and cell culture will have an impact on the strength and elasticity of the final engineered construct. It is possible to create tissue engineered material for urethroplasty.
