Abstract

The aim of this study was to evaluate the efficacy of polidocanol as a sclerosing agent for varicose veins of the lower limb, as regard cosmetic results, incidence of complications and a achievement of patients satisfaction of the results, besides evaluation of a modification of the technique of injection and weather it will achieve superior results over the classical technique or not.

Sixty patients were included in this study (4 males and 56 female) they were all diagnosed to have varicose veins of the lower limb with no saphenofemoral or saphenopopliteal reflux by duplex ultrasound. 4 patients with incompetent perforators (IPs) were included in the study after legation of the IPs. and the patients were divided into two groups each group included 30 patients.
The mean age (± SD) in group A was 28.5 ± 8.4, and in group B was 29.4 ± 6.7. The risk factors were nearly similar in the two groups as regard hereditary factors, previous pregnancies, previous DVT or occupational long standing up.
All patients were complaining of disfigurement attributed to the presence of varicose veins in their lower limbs , 24 patients (40%) were complaining of sense of pain or soreness on long standing where 13 patients fell in group A (43.3% of group A) and 11 patients fell in group B (36.7% of group B) ,19 patients (31.7% ) were complaining of heavy legs 10 patients were in group A(33.3% of group A) and 9 patients in group B (30 % of group B) , two patients (3.3%) were complaining of varicose veins with previous rupture varicose one patients was included in each group and 9 patients (15 %) were presented with residual varicose veins after surgery , 5 patients was in group A and 4 patients was in group B.
As regard the types and sites of concerned veins all patients were having elements of spider veins (100%),45 patients (72%) were having reticular veins 21 patients in group A and 24 patients in group B , truncal or main tributaries varicose were present in 24 patients (40%) where 10 patients fell in group A and 14 patients fell in group B. 45 patients (75%) were presented with veins in the leg , at the knee region and in the thigh where 25 patients fell in group A and 20 patients in group B , 10 patients (16.7%) were presented with veins at the knee region 3 patients in group A and 7 patients in group B and 5 patients (8.3%) were presented with reticular and spider veins confined to the ankle regions 2 patients in group A and 3 patients were in group B .

There were bilateral varicose veins in 23 patients (38.3%) 10 patients in group A and 13 patients in group B.
We found no statistical significant difference between the two groups, as regards the risk factors, complaint or the clinical presentation.
