Abstract

The terrible triad injury of the elbow is a fracture and dislocation of the elbow joint. In particular, the elbow is dislocated in a posterior direction. The coronoid bone is fractured. The third part of the injury is a fracture of the radial head. This term is used because of high rate of complications that often occure (Ilyas and Jupiter; 2008).
Despite the complexities of this injury, an understanding of the relevant anatomy, the mechanism of elbow injury, the primary and secondary constraints providing stability, the soft tissue injury patterns, and better methods of surgical repair led to develop a consistent surgical strategy for these injuries. This includes (1) fixation or replacement of the radial head, (2) fixation of the coronoid fragment, (3) repair of the lateral collateral ligament complex, and (4) repair of the medial collateral ligament, with application of a hinged external fixator for patients who demonstrate residual instability. This protocol restores congruent elbow stability, allows early motion, enhances functional outcome, and minimizes complications (McKee et al; 2005).

However, despite the best attempts at reconstruction, even in those experienced with treating these injuries, the final outcome may be fair or poor, according to reporting of several clinical series. Further research is required to improve the outcome of these serious injuries (Ring et al; 2002).
