Abstract

The emergence of bacterial resistance to different antimicrobial agents has been reported with increase frequency world wide .The most common predisposing factor is the missuse of antibiotics especially B-lactam antibiotics , and the most common mechanism of bacterial resistance to these agents is inactivation by B-lactamases.Extended spectrum B-lactams were able to control this resistance until evolution of ESBLs producers especially among klebsiella iolates conferring resistance to all extended spectrum cephalosporins. The problem of ESBLs is still underestimated world
wide as routine susceptibility tests which are currently used cannot predict ESBLs production .The aim of this study is to determine a simple and reliable
method that could detect ESBLs producing strains of Klebsiella in routine clinical laboratory.To reach this goal 44 clinical isolates of klebsiella were collected from patients admitted to different intensive care units of Menofia university hospital .These isolates were identified by cultural and biochemical characters.They were tested for susceptibility to third generation cephalosporin then the resistant were tested for ESBLs confirmation by disc diffusion using (CAZ ,CTX and CPD with and without clavulonic acid ) , E test using ( CAZ & CAZ + CA )and chromogenic substrate (strip method).
