Abstract The aim of this study is to discuss different ways of management of hydatid cysts of the liver and lung including, diagnosis, ,different surgical techniques, postoperative care and management of complications. Background: Hydatid disease or ecchinococcosis is azoonotic infection caused by larval stages of cestode species of the genus ecchinoccocus(atape worm), cystic form is caused by Echinococcus granulosus, alveolar form is caused by E.multilocularis,and polycystic forms are caused by either E.vogeli or E.oligarthrus. The disease is common in the sheep-raising Mediterranean regions, Australia, portions of Africa, India, Turkey, Southern Europe, New Zealand and very uncommon in United States. Infestation by hydatid disease in human most commonly occurs in the liver (55-70%) followed by the lung (18-35%); the two organs can be affected simultaneously in about (5-13%). Other organs can be affected as heart, brain, bone and spine. Clinical presentation of hydatid disease depends on the site and size of lesion. Diagnosis is made ultrasonically, chest X-ray, CT scan, MRI are of great value in diagnosing cardiac and intracranial hydatiosis. Different serological tests are carried out for the diagnosis,screening and post-operative follow up for recurrence. Surgery remains mainstay of treatment,with less invasive methods and combinations with chemotherapy coming to the fore as techniques develop. Surgical management of a pulmonary hydatid disease include the complete evacuation of the cyst with the endocyst and the avoidance of the contamination and management of residual cavity. Normal lung must be preserved as much as possible. For cysts with abscesses or severe pulmonary destruction, lobectomy or even pneumonectomy is required. Control of the E. granulosis includes long-term measures of public health education with primary health care and veterinary public health activaties,such as improvement of slaughter hygiene and meat inspection ,dog registration and sanitation measures.
