Abstract

The evaluation of acute chest pain remains challenging . As AMI is one of the serious causes of acute chest pain so must be diagnosed effectively. Patients with AMI who are mistakenly discharged from the emergency department have mortality rates of about 25% .On the other hand the inappropriate admission of cases of acute chest pain costs constitute the largest category of losses in the emergency department .The diagnosis of cases of AMI without electrocardiographic changes represent a challenge. To guide decision making with regard to these patients attention has focused on the use of macromolecular markers (CK-MB , cardiac troponin I (cTn-I) , myoglobin and H-FABP) for the evaluation of chest pain but there is confusion about the optimal use of these markers . The most appropriate strategy for the combined use of available markers remains uncertain. At many hospitals, assays for both CK-MB and cardiac troponins are routinely ordered for all patients with acute chest pain .Most recently, H-FABP is introduced as an early marker of diagnosis of AMI cases who present with recent onset of chest pain without typical changes in the ECG, as H-FABP can be detected much as early as 20 minutes following AMI .
