Abstract

Screening for thyroid hormone (TH) levels in the first week of life is extremely important to identify infants with congenital hypothyroidism (CH). Worldwide neonatal screening programs have been successful in decreasing childhood mental retardation (MR) related to CH by early detection and treatment .With screening, developmental prognosis is considerably improved, but follow-up studies still report growth and mental delay compared to controls .The primary objective of the present study was to compare children with CH aged three years and more diagnosed and managed by Egyptian Newborn Screening Program at Alexandria Governorate with their similar unaffected children at the same age, sex, social classes to explore differences if present in growth percentiles ,social adaptive behavior and Intelligence scoring .
Comparative cross sectional study was carried out to compare between the selected samples(the differences of intellectual functions ,social and adaptive behavior levels between early detected and treated children with CH and other children similar for age ,sex and socio-economic level who don’t have CH or suffering from MR due to any other causes). The present study included 58 children representing all cases of CH children aged 3years and above who had been detected early through the NSP and on regular treatment at the health insurance centre that following MOHP .For comparison,170 children were selected as control group who were similar for age ,sex and socioeconomic level of the patient’s group .Children were collected from outpatient clinics from both Wingate (El-Raml) Pediatric Hospital and Sporting Students Insurance Hospital(SSIH).
Data was collected in the pre-designed questionnaire and analyzed using the relevant statistical methods at confidence level 95% and using SPSS program version13.
The following results were obtained: Regarding the general characteristics of the collected sample, it was found that the percentage frequency of female sex among the cases was higher than male sex. The male sex among cases constituted 43.1% compared to 50.6% in control group, while female sex among cases (56.9%) were more than control group (49.4%) .However the difference was not statistically significant. The present study failed to show any association, between the frequencies of consanguineous marriages among both cases and control.
No significant difference between both cases and controls was found regarding maternal age, paternal age, parental consanguinity, maternal parent’s consanguinity and paternal parent’s consanguinity. Regarding the social level, there was significant difference between both cases and control groups in high level and low middle levels while there was no significant difference in both high middle and low social levels. In addition, there were no significant differences in the weight, height while significant difference was found in head circumference percentiles of both cases and control groups.
