Abstract

Glaucoma that is present from birth or shortly after is caused by improper development of the anterior chamber angle in primary developmental glaucoma or by damage to the aqueous outflow system by maldevelopment of some other portion of the eye in secondary developmental glaucoma.
Ocular enlargement occurs because the neonatal globe is still distensible; the corneal and sclera collagen have not hardened sufficiently to prevent their expansion with increased intraocular pressure. This change includes stretching in all parts of the infant eye: cornea, anterior chamber angle, sclera, optic nerve, sclera canal, and lamina cribrosa. The increased IOP stretches also the corneal endothelium and descemet’s membrane resulting in breaks in these layers.
The sclera also expands slowly under the influence of elevated IOP resulting in sclera thinning and increased visibility of the underlying choroid in the neonate and a-blue sclera-appearance, in addition the axial length of the globe increases resulting in myopia and astigmatism.
The delay in diagnosis results in corneal opacity that obscures the visualization of the anterior chamber angle which renders the surgical intervention more difficult.
It was shown that we should have a screening program for the neonates to detect those with congenital glaucoma, as early diagnosis and proper surgical intervention at this stage have a great impact on the long term visual outcome of these patients.
