Abstract

the difficulty we still have in discontinuing mechanical ventilation evidenced by the fact that 40% of the time that a patient spends receiving mechanical ventilation is devoted to weaning, whereas, in COPD patients this rate increases up to 59%.
A large spectrum of weaning indices is used. Most of these indices used based on measurements made solely during mechanical ventilation or spontaneous breathing trial or both working for weaning assessment performed during spontaneous breathing.
Studies have focused on respiratory mechanics during the spontaneous breathing trial (SBT), but little attention was given to the physiologic assessment after imposed work of the SBT, or after the institution of the ventilatory support, and reversal of the sedating effect that may provide insight into the respiratory reserve required to sustain spontaneous breathing and predict weaning outcome.
By evaluating V˙E and its components as a trend immediately following the institution of ventilatory support, it is hypothesized that the recovery time to baseline V˙E, Minute ventilation recovery time (MVRT) may be a practical measure of the ability to predict weaning outcome. A longer recovery time felt to represent decreased respiratory reserve and inability to maintain supported breathing.
Therefore prospective study for evaluating this respiratory parameter (minute ventilation) V˙E and minute ventilation recovery time, in COPD
Patients with respiratory failure, undergoing ventilatory support. Minute ventilation recovery time (MVRT) has recently shown to potentially predict weaning outcome, and defined as the time required for minute ventilation (V˙E) to return to the baseline (MVRT) considered a promising index for the clinical assessment of respiratory reserve as they convalesce from respiratory failure, and thus potentiality very useful as a weaning parameter.
