Abstract

mitral annulus systolic velocity measured by Tissue Doppler Imaging (TDI) offers an alternate method for assessment of global left ventricular systolic function. However, there has been no study correlating mitral annulus systolic time intervals with left ventricular ejection fraction (LVEF).
Aim of the work: To detect the possible role of mitral annulus systolic time intervals assessment by Transthoracic Echocardiography (TTE) and Tissue Doppler Echocardiography (TDE) in measuring LV systolic function in some heart diseases including ischemic heart disease, hypertension, cardiomyopathy and valvular heart disease.
Methods: This study included 106 patients (57 males and 49 females) with mean age of 53.8 ± 11.6 years and who were admitted to CCU and General Ward of Cardiology Department of Zagazig University Hospitals. Patients were grouped into six groups:
Group I: Acute Coronary Syndrome patients (ACS, 36 cases); subgrouped into: Subgroup IA: Unstable Angina (UA, 14 cases), Subgroup IB: Non-ST Elevation Myocardial Infarction (NSTEMI, 10 cases)& Subgroup IC: ST-segment Elevation Myocardial Infarction (STEMI, 10 cases). Group II: Ischemic Cardiomyopathy (ICM, 10 cases). Group III: Dilated Cardiomyopathy (DCM, 10 cases). Group IV: Hypertensive heart disease (HTN, 10 cases). Group V: Valvular heart disease (30 cases); subgrouped into: Subgroup VA: Mitral Valve Disease (MVD, 10 cases), Subgroup VB: Aortic Valve Disease (AVD, 10 cases), Subgroup VC: Combined valve disease (10 cases) and Group VI: Control group (10 cases).
Initially, a TTE was performed, and LVEF was determined using the M. Mode and the modified Simpson`s method. Then, the system was swithed to TDI velocity mode. Apical 4- and 2-chamber views were displayed with Doppler sample volume placed respectively within the lateral, septal, anterior and inferior corners of the mitral annulus for spectrum sampling.
For the measurement of timing, the beginning of QRS complex was used as the reference point. Time to peak of the systolic mitral annular wave (TS) and regional Pre-Ejection Period (PEP) were measured. The mean time intervals at the four mitral annular sites (lateral and septal, anterior and inferior) were calculated to show if they can be used as indices for quantitative assessment of global systolic function by doing correlation between TS, PEP and LVEF.
