Abstract Despite the decrease in the over all incidence of complications after CABG ,yet the incidence of postoperative strokes remains the same and the risk is a direct function of the patient’s age ranging from 0.2 % in patients less than 45 years old and up to 8% in patients above 75 years old, where the current increase in the mean age of CABG patients highlights the stroke risk. The causes of perioperative stroke are multifactorial but mainly: 1. Aortic causes: Due to iatrogenic mobilization of calcified aortic plaques (during cannulation or cross-clamping ) leading to cerebral embolization. 2. Carotid stenosis: Which is the only anatomic situation about which the surgeon can routinely take action to remove the pathology (hence its importance). 3. Other causes: Air or fat embolism , peripheral vascular disease (PVD), hypothermia , hemodynamic fluctuations ,systemic inflammatory response …etc Cost of perioperative stroke: Post operative stroke remains a catastrophic and costly complication with up to 10 fold increase in mortality and 3 fold increase in hospital stay as compared to non-complicated patients. |
