Abstract

knee flexion for more than 120 ͦ is important for many daily activities especially in our Arab countries. Limitation of flexion after total knee arthroplasty may result in patient dissatisfaction after total knee arthroplasty especially for patients with a good pre-operative range of motion.
In this study we tried to evaluate the early functional results of high flexion total knee prosthesis in comparison with conventional total knee arthroplasty. It is non-randomized prospective study done on 60 patients all of them complaining of disabling degenerative osteoarthritis of the knee. 30 patients treated with high flexion total knee prosthesis (Nex-Gen LPS Flex) and another 30 patients treated with conventional total knee prosthesis (Nex-Gen LPS).
The mean pre-operative maximum knee flexion in high flex knee group was 123.31 ͦ while in conventional knee group was 99.601 ͦ.The mean follow up period was 14.96 months. At last follow up of the patients the mean post-operative range of motion in high flex knee group was 125.2 ͦ with no significant obvious improvement than the pre-operative range of motion while in conventional knee the average range of motion 118.939 ͦ. The knee society scoring system used for pre- and post-operative assessment. The mean pre-operative knee score in high flex knee was 31.965 and the mean pre-operative functional score was 38.64 while in conventional knee group the mean pre-operative knee score was 31.7 and the mean pre-operative functional score was 37.145. In high flex knee group the mean post-operative knee score was 78.865 which is considered good result. And the mean post-operative functional score was 85.35 which is considered excellent result while in conventional knee group the mean post-operative knee society score was 78.31 and the post-operative functional score was 83.285 which also considered good results.
These results indicate no significant difference between both groups with post-operative improvement in all cases. But long term follow up must be considered later on for proper evaluation of durability high flexion total knee arthroplasty and conventional knee with further concentration on loosening and polyethylene wear.
