Abstract
Introduction: Laparoscopy is now regarded as an effective tool for diagnosing and staging malignancy. Also, the use of laparoscopy in the palliation of abdominal cancer may be one of the best applications of minimally invasive surgery.
Aim of the work: This study is constructed to clarify the value of laparoscope in assessment and staging of cancer and different techniques of laparoscopic palliation.
Staging laparoscopy has proved great efficiency in gastrointestinal malignancies, especially those used to present late as gastric, pancreatic & esophageal cancers. This avoids about half the number of non therapeutic exploratory laparotomy in some studies, although controversy about exact timing and role still exist.
Palliation is considered an integral part of management of cancer patients and fundamental part of practice of surgical oncologists. Those especially ill patients in most circumstances cannot withstand the high morbidity of open surgery where laparoscopy and laparo-endoscopy can provide a better chance for them with lesser complications. Commonly practiced procedures include: lap jejenostomy, double bypass for pancreatic head cancer patients, palliative stoma and thoracoscopic splanchinectomy for pain.
Currently robotic surgery is being studied by large number of centers as a tool for palliation and even resection of malignant tumors. It still early to comment on its valuability in comparison to laparoscopy in these issues.
Conclusion: Laparoscopy is an effective tool both in assessment of cancer patients and in palliation of advanced cancer with promising results in experienced hands with minimal complications.
