Abstract

This study aimed to study rate of utilization of family health services and factor affecting utilization through across section study. The southern Shebine El-Kom family health center and Kafer Tanbedy family health unit were selected through multistage random sample technique. The southern Shebine El-Kom family health center represented the urban area while Kafer Tanbedy family health unit represented rural area.
Kafer Tanbedy Provides services for 13887 persons from all age according to it’s statically data. Shebin El-Kom family health center provides services for (77957) persons from all age according to the statistical data of local health authorities of Shebine EL-Kom city.
Measure response of attendants in both areas in unit and center we founded that: as regarding socioeconomic character of randomized attendants 1035 represent total utilization of family services in unit while 4155 in center.
Mainly of age of attendants in both rural and urban area were 15 83.7%in unit and81.9% in center, regarding sex it was mainly of female 85, 4%. Recurrent visits represented 72.8% and 66.1% in rural and urban facility respectively. Married patient represented 88.4% and 88.2% in unit and center respectively in rural and urban area.
As regard education level secondary level represented 54% in unit and 45%in center Unemployed were higher represented 55% and 85% of attendants in rural health unit and urban health center. There is statistically significant more frequent in rural than urban as regard Residence, Martial state, Education and urban.
In these study 40.5%of children utilized the curative services in the Kafer Tanbedy health unit while 62.4%of children utilized the vaccination services in the Southern family health center. Also there is statistically significant (0.001) in rural than urban as regard curative and follow up visit .while it is higher in urban than rural as regard child vaccination and thyroid function test.
Total number of children in unit and center were 6506and 42.701 respectively.40.5%and 18.9% represents number of curative service provide for children in both unit and center,34,1%and 60% represent number of child vaccination of total child number in unit and center . 5.7%and 9.5% represent number of thyroid function test in unit and center As for natal care services home visit provided for 82.5%, 53.4% in unit and center respectively while vaccination were 50.9%64.7% in unit, and center respectively .regarding laboratory service: pregnancy test performed for 35.1%and 44.8% in both unit and respectively .Their is statistically non significant difference (>0.05) between rural and urban health facilities as regard all services provided for pregnant women except for vaccination their significant difference urban more than rural (<0.05).br>The postnatal care services were 145 and 63 in rural and urban area respectively. First visit represented 41.3%, 44.4% in rural and urban area respectively. While 31.1% and 30.1% represented the second visit. Their is no statistically significant difference between rural and urban.
Curative service provided for adult in unit and center were 100%.laboratory services divided into urine & stool analysis with 68.9%and 72% respectively in unit and center. Their no statistically significant (>0.05) as regard curative, laboratory services except for blood group (<0.001) their statistically significant in urban than rural area.
Elderly person in rural and urban area as follow 670 and 4209 person, 100% represent curative services provided 72% of women attended to Kafer Tanbedy unit used oral contraception. 33%of women attendant to Southern family health center used IUD, follow with 29% injection one.
As regard response of randomized attendants were as follow:
Response of attendants were significantly higher in rural family health unit than urban family center as regard methods of administration ,doctor and nurse dealing, health education, laboratory services and pharmacological services. Expect for easiness of transportation, waiting time and sufficient chair the satisfaction for them were higher in urban than rural one.
