Abstract This study was conducted in Ain Shams University Maternity Hospital in the time between October 2009 to Jully 2010. The objective of this study was to measure aspartate aminotransferase (AST) and alanine aminotransferase (ALT) enzymes levels in vaginal washing fluid in order to detect preterm premature rupture of membranes. A total of 74 pregnant women were included in this study, between 26-36 weeks of gestation, and were divided into two groups: - Group I (case group): 37 pregnant women with confirmed PPROM group (Cases group). - Group II (control group): 37 normal pregnant women. The posterior vaginal fornix was irrigated with 3ml of sterile saline using a sterile Cusco’s speculum and 5ml of the irrigated saline was aspirated with the same syringe and the fluid specimens were sent immediately to the laboratory for assay of AST and ALT, After confirming the absence of amniotic fluid pooling and blood in the posterior fornix for the control group, the sampling method and AST and ALT assays were be performed as previously described for PPROM pregnant women. Vaginal AST and ALT were lower in control group than case group and the differences were statistically highly significant (p value <0.001). The best cut-off value for vaginal AST concentration for detection of PPROM was 7.5 IU/l (sensitivity 91.9%, specificity 91.9%) while the best cutoff value for vaginal ALT concentration for detection of PPROM, was 1.5 IU/l (sensitivity 91.9%, specificity 89.2%). |
