Abstract

Impairment of renal function is common among elderly patients due to an age-related decline in renal excretory function. Improvement in end-of-life-care is required for patients with chronic kidney disease (CKD). The aim of the study was to apply palliative care planning for the most common problems for patients undergoing Hemodialysis. Research design: A quasi experimental research design was used. Setting: The study conducted in hemodialysis center at Hodiedah city. The purposive sample composed of fifty patients. Tools: First tool was an interviewing questionnaire sheet part used to collect data about patient`s socio-demographic characteristics, health history part to consists of second tool about dialysis symptoms index to determine most common symptoms. According to patient`s assessment (pre intervention), the researcher detected most common symptoms, then develop palliative care planning and implemented through session. Result: A total of 50 patient about three quarters of them were male, with more than two fifths of them more than 50 years, and most of them were not attendance of training programs. The most common causes of renal failure were infection (42%), congenital deformity (40%) and hypertension (34%). The most common symptoms as reported by patients were fatigue (78%), itching (74%) and cramps (64%). The results showed statistical significant difference between cramps burden related palliative care plan (0.006), and there are significant statistical relations in severity of most common, total of their, and total of dialysis symptoms. Conclusion: Majority of patients had physical and psychological problems different in its burden and severity from patient to other and the most common problems were fatigue then itching and muscles cramps. The palliative care plan had positive impact on patients undergoing hemodialysis also decrease and control burden and severity of most common problems in patients undergoing hemodialysis.
