Abstract

The present study was done to evaluate demographics, clinical manifestations, imaging criteria, therapeutic interventions and outcomes of 100 diabetic patients (admitted to the Diabetic Foot Unit at the Main University Hospital, University of Alexandria).
The prevalence of DFI among the patients was 61% males and 39% females. PAD was found to be significantly more common in diabetic males. The prevalence of PAD (diagnosed by ABI<0.9) among diabetics of different age groups was 38%. The incidence of PAD among diabetics increased with increasing age where most PAD patients were above 50 years of age and the incidence of PAD jumped from 9% in diabetic patients <50 years of age to reach 46.1% in diabetics >50 years. Also, PAD incidence increased with longer duration of diabetes.
Smoking, hypertension, and hyperlipidemia were insignificant high risk factors for development of PAD in diabetics.
No statistically significant difference was found between PAD group and non-PAD group as regards the type of diabetes or the mode of therapy used for control of diabetes.
Among the associated co-morbid conditions screened for in the present study, coronary vascular disease (CVD) was found significantly higher among PAD group while both retinopathy and nephropathy were found more significantly associated with non-PAD group.
Symptoms of neuropathy were present in 46% of patients. The most frequent symptom detected was numbness. Testing for neuropathy done using monofilament test, vibration sense, and ankle reflex showed that vibration sense testing had the highest sensitivity (76%) while monofilament testing had the highest specificity (81%).
Among the studied group of patients, 10 of them (10%) required only antibiotics and dressings, while 90 of them (90%) required surgical intervention, 38 of them required angioplasty (38%) before surgical intervention.
