Abstract T he present study was designed to evaluate the level of Cathelicidin (LL37) in congenital pneumonia and to evaluate its relation to (25 OH)D status and its diagnostic and prognostic value in these cases. We have recruited 60 full-term neonates from neonatal intensive care unit of Shoubra General Hospital from the period between 11\2011 till 4\2012. They were divided into 2 groups: Group 1: full term Neonates were diagnosed to have congenital pneumonia (cases group). Group 2: healthy neonates as a control group (30 patients). All the patients were subjected to History taking, clinical examination, Laboratory investigations (Complete blood count (CBC) with differential leucocytic count, CRP, Blood culture,, Measurement of serum 25(OH)D and Measurement of human plasma LL37). The patient group included 30 newborns; 14males (46.7%) and 16 females (53.3%), with mean gestational age of (37.1±0.305 weeks), mean birth weight of (2.81±0.1947k g), mean serum LL37 level of (47.00 ±22.9pg/mmol), Mean 25(OH)(34.833 ±18.16 ng ml). A control group of 30 healthy neonates [15 males (50%) and 15 females (50%)], with mean gestational age (37.4667±0.57 wks), mean birth weight (2.8900±0.17Kg), mean plasma Cathelicidin (LL37) level (8.533±4.977Pg/mmol), Mean 25(OH) D (93.2000±36.1222 ng ml). All patients in the study were subjected to adequate history taking, full clinical examination, CBC, CRP with titre, blood culture and plasma Cathelicidin (LL37) level and serum 25(OH) level. Significant risk factors for congenital pneumonia included increased duration of PROM, Maternal fever. Neonates with congenital pneumonia had their plasma Cathelicidin (LL37) levels significantly higher than those of the control group.25(OH)D level was lower in in neonates with congenital pneumonia than control group. There was a significant negative correlation between basal Cathelicidin (LL37) and Apgar score at 1 and 5 minutes cases group. There was a significant positive correlation between basal Cathelicidin (LL37) level and length of stay in cases group. There was no correlation found between Cathelicidin (LL37) and 25(OH) in both groups. LL37 level was reliable to detect congenital pneumonia P <0.0001. The cut off value was 17 pg/mmol with 93% sensitivity and 86% specificity. |
