Abstract B enign prostatic hyperplasia is an exceedingly common condition which affects most elderly men. The unpleasant and unacceptable symptoms of this diseases are initiative of an important health, economic and social problem. Traditionally, surgery is a definitive treatment; but due to the accompanying pressures on the patient , the treating surgeon and hospital services, and an effective medication to ameliorate the symptoms is looked forward. Some authors reporting a beneficial effect have evaluated many phytotheraputic agents, while others have found them to be ineffective. The aim of this work was evaluation of the use of phytothearputic agents for treatment of benign prostatic hyperplasia. this work included a review of the pathology, clinical picture, clinical assessment of the disease; different option for treatment; and different phytotheraputic agents. phytotherapeutic agent has gained wide spread worldwide with trading exceeding billions of dollars due to several factors their safety & easy accessibility. Serenoa repens (Saw Palmetto Berry) (( Permixon )) The most popular phytotherapeutic agent available for the treatment of BPH. The most widely suggested mechanism of action of S. repens is as a 5α-reductase inhibitor. A recent published study showing S.repens improved symptoms, improved urodynamic parameters but did not significantly change prostate volume. Furthermore it demonstrated an excellent tolerability with no impairment of sexual function or other side effects. Finally, it did not interfere with cancer screening markers since PSA remained unchanged. Patients’ satisfaction measured in Romanian and Slovak studies was about 90 %. Pygeum africanum (African Plum) (( Tadenan )) The postulated active components include phytosterols, especially b-sitosterols, pentacyclic triterpenoids and esters of long-chain fatty alcohols. Pygeum africanum extract may suppress LUTS by reducing bladder hyperactivity, decreasing inflammation, and protecting against abnormal prostate growth. Hypoxis rooperi (South African Star Grass) This extract mainly contains beta-sitosterol (Harzol), along with various amounts of several other sterols. Beta-sitosterol is thought to be a major active component in plant extracts. Their conclusion was that β-sitosterol does improve urologic symptoms and urinary flow rates, but its long-term effectiveness, safety, and ability to prevent the complications of BPH are unknown.
