Abstract

In this study we investigated the expression of CD200 on neoplastic cells of patients affected by MCL and B-CLL patients retrospectively by immunohistochemistry on paraffin embedded Lowy-fixed bone marrow (BM) trephine biopsies and the correlation between CD200 and clinical and laboratory findings in CLL patients .
To achieve this aim, this study was conducted on 61 patients, 48 patients with CLL and 13 patients with MCLwho were attending the cancer institute of south vally during the period between 2005 to 2012 besides 15 healthy subjects age and sex matched as a control group.
Inclusion criteria:
well defined diagnosis of B-CLL and MCL.
Exclusion criteria:
Patients with Liver dysfunction, other CD5+ve lymphoproliferative disease or malignancy and autoimmune disease.
Patients and methods:
Retrospective data collected include:
• Full history taking with complete clinical examination stressing on anaemic manifestation, bleeding tendency, B-symptoms, presence of liver enlargement, splenomegaly and lymphadenopathy.
• The following investigation:
- CBC : white blood count (total and differential), Hgb, platelets.
-coomb’stest:
- lactate dehydrogenase (LDH):
- uric acid:
- Bone marrow aspirate and immunophenotyping(CD23,FMC7, CD22, sIg, CD5):
-Bone marrow biobsy was taken on admmision before treatment:
o Detection of t(11:14) is made by Fluorescent in situ hybridization (FISH).
o Detection of cd200 is made by Immunohistochemistry using a rabbit anti-human CD200 IgG affinity purified polyclonal antibody (SIGMA,U.S.A.) number HPA03119.
Our results show that:
a) Cd200 expression was significantly higher (p1<0.001) in CLL(47.4±28.9) patients compared to MCL (6.2± 5.3)and control (8.7 ± 2.1).
b) But no significant difference could be detected between MCL and control group.
c) the ability of the test to detect positive cases (sensitivity) was 72.9 % and the ability to detect negative cases (specificity) was 92.3 %.The ability to predect positive cases(PPV) was 97.2 % and that to predect negative cases (NPV) was 48 %.The test was accurat in 77 % of cases.
d) No correlation betweenCD200 expression and different staging system of CLL patients.
e) No correlation could be between CD200 expression and wbcs ,Hgb, platelets, LDH , percentage of bone marrow infilteration and uric acid levels (p> 0.05).
