Abstract

Osteoarthritis is the most common disease of joints and represents a huge burden to society in terms of personal suffering and health resource utilization.
The aim of this work is to compare between early result of hyaluronic acid injection in the moderate osteoarthritis of the knee, and diacerein drug administration.
The study included 60 patients presented with moderate knee osteoarthritis were subjected to complete physical examination and radiological studies, the radiographic views will be obtained and studied as standing antero-posterior, lateral and skyline views.
The patients were divided into two groups, the first group: include thirty patients treated with hyaluronic acid injection (low molecular weight sodium hyaluronate 500-750,000 kilodalton with concentration 20mg/2ml) which is these patients will be given 3 injections at weekly interval.
The second group; include thirty patient treated with diacerein administration. These patients will be given diacerein (50mg) twice daily for 3 months.
Both groups, were allowed to use NSAID for the first 2 weeks. Patients were followed up for at least 6 months and assessed at the end of this period.
At the end of the follow up period, according to chondral defect scoring system, the mean age for the study was 62.50 +7.33 years, there were 39 females and 21 males.
The final score of diacerein medication was, 9 patients had excellent results, 17 patients had good results and 4 patients had poor results.
The final score of hyaluronic acid injection was, 7 patients had excellent results, 18 patients had good results, 1 patient had fair result and 4 patients had poor results.
All subjective and objective complaint were significantly improved as regarding there premedications scores. There was negative correlation between the age and the final outcomes.
There was evident improvement in the pain, ability to perform the work and ability to squatting for diacerein adminsteration over hyaluronic acid injection.
There was mild improvement in the swelling, locking and range of motion for hyaluronic acid injection over diacerein adminsteration, but this was unsignificant.
The side effects of diacerein medication is higher than hyaluronic acid injection.
