Abstract

Diabetes mellitus is the most common endocrine disease and is associated with organ complications due to micro vascular and macro vascular disease. The incidence of DM has increased dramatically in recent decades, predominantly because of changes in life style, an increase in the prevalence of obesity and longevity. Individuals with diabetes mellitus are reported to have increased risk of UTI both in frequency and severity. Patients with DM have a higher prevalence of asymptomatic bacteriuria and a higher incidence of symptomatic UTIs, which more often lead to complications compared with those without DM. This study was conducted in Damietta cardiac hospital in order to estimate the prevalence of urinary tract infection among diabetic patients in Damietta. It include one thousand individuals ( 500 diabetic and 500 non-diabetic ). The included individuals were clinically evaluated, including search for potential infections and history taking was done for each patient. • The initial maximum temperature was recorded for each patient. • Body mass index was used to gauge obesity, which is equal to weight/height2 (in kg/m2). BMI of 30 is most commonly used as a threshold for obesity in both men and women. • Random blood sugar was recorded for each patient. • Total leukocyte count and cell differential was recorded for each patients. • Complete urine analysis was performed. • Serum createnine was recorded for each patient. By analyzing and processing the data obtained from the history, clinical examination and lab work the study declared that: The prevalence of urinary tract infection (UTI) was 52.2% in diabetic group which was significantly higher than prevalence of UTI in non diabetic group 22.4%. UTI appears to be multifactorial in patients with diabetes and various diabetes-related risk factors have been proposed including history of previous UTI, type of diabetes, longer duration of diabetes and poor glycemic control. E.coli was the most frequent uropathogens in both diabetic and non diabetic group. Most of the isolates showed intermediate to low level of resistance to one or more antimicrobials tested. This indicates that regular monitoring is required to establish reliable information about resistance pattern of urinary pathogens for optimal empirical therapy of diabetic patients with UTI.
