Abstract

Introduction: The writing of correct, precise prescriptions by physicians is an art learned though long practice and training. It is governed by an array of factors, including the physician, the practice setting and the health delivery system. Aim: the present study was conducted to assess the family physicians’ prescribing patterns in pediatrics using WHO-INRUD(international network for rational use of drugs) and IMCI guidelines and To detect the family physician’s reported barriers regarding proper prescribing in pediatrics.
The study aims at improving the quality of pediatrics care provided by Family Physicians in Family practice Centers, Suez Canal University Subjects and methods: The present study is a cross sectional analytic study that was carried out in Family Medicine Practice Centers (FMC) affiliated to Suez Canal University Hospitals distributed in Ismailia, Suez & Port Said. The study sample included all the registered family physicians working in these centers(n=50).
The study tools were a questionnaire that was distributed among the participants and assessment of all prescriptions for under five children over a month (N=585).
SPSS program was used for data entry and statistical analysis.
Results: according to the results of the study85.6% of family physicians had low quality of prescriptions in pediatrics while only14.4% had high quality of prescription . 98% of physicians felt that they need more knowledge and training. qualification (p= 0.000)and practice location in FMC (p=0.000), were a highly significant variables in assessing the prescriptions’ quality. received training was a statistically significant variable(p=0.003) in assessing the prescriptions’ quality while no statistical significant difference between other socio-demographic characteristics.
The average number of drugs prescribed per patient ranged from (1 to 4 drug per patient) that exceeded the recommended average from WHO (1.6-1.8 drug per patient) and the percentage of prescriptions with antibiotics was 52.8% that exceeded the recommended level by WHO (20-30%) and this indicate over prescribing of antibiotics by physicians in this age group.
Also it was found that the percentage of drug prescribed by generic name was very small (0.9%) while the percentage of drug prescribed from essential drug list was 85.5%.
Training courses (54%), availability of standard guidelines (32%) ,availability of specialized programs (21%) and availability of group discussion in scientific day (12%) came at the top of physicians’ recommendations
.
Conclusion: this study addresses areas of improvement that might be a stimulus to design a tailored educational intervention in order to improve the family physician’s prescribing patterns in pediatrics.
