Abstract

This study aimed to : Shift work is defined as work outside of daytime hours. Shift work has been associated with gastrointestinal disease, cardiovascular disease, diabetes, several types of cancer, and metabolic disorders. The mechanisms linking shift work to health problems are not clear, but changed circadian rhythm, sleep problems, stress, and lifestyle and behavioral changes such as diet and physical activity might be potential mediators. Aim The current study aim to describe the adverse effects of shift work on the physical and psychological health of nurses, sleep disorders and the effect of shift on social aspect of the nurses in an attempt to provide applicable recommendation to decrease stress produced by working condition and promotes the pest patient care. Subjects and methods A comparative cross sectional study was conducted upon (150) shift nurses and a matched comparative group of (150) day time nurses from Mansoura University Hospitals. The studied groups were interviewed using structured questionnaire. Anthropometric measurements including height, weight, waist circumference and BMI were done. Blood pressure of studied group was measured. Result There was statistically significant as regard gastrointestinal disorder symptoms ,i.e. disturbed appetite, nausea, constipation or diarrhea and stomach upset among shift nurses compared to day time nurses .As regard cardiovascular disorder symptoms, It was found that heart palpitation, irregular heart beats; chest pain and dizziness were more frequent among shift work nurses compared to day time nurses. Regarding the pattern of menstrual cycle among studied group, there was no statistically significant different (p 0.05), before starting the work. However menstrual cycle pattern have been changed since starting the work to become more frequently irregular among shift work nurses (66.6%) compared to day time nurses(14%) with statistically significant difference (p 0.001). Current research results have reported that 49.3% of shift nurses were obese, while 27.3% of day time nurses were obese. The difference between the two groups were statistically significant (p 0.001). The metabolic disorders criteria were more frequent among shift nurses compared to day time nurses with statistically significant differenc. Regarding the level of burnout subscales, emotional exhaustion and depersonalization were statistically significant higher among shift nurses compared to day time nurses. Concerning depression and anxiety, the shift nurses had higher level of anxiety and depression compared to day time nurses with statistically significant different(p 0.001). The percentage of shift nurses suffering from poor sleep quality was 84% compared to 26.7% among day time nurses with statistically significant difference (p 0.001). This study showed that, the frequency of needle stick and sharp objects injuries were more frequent among shift work nurses in comparison to day time nurses with statistically significant difference. Based on the previous result, this study recommended the following measure 1. Shift Schedule Design 2-Sleep regulation 3-Dietary and eating pattern and 4-Social activities.
