Abstract Contraceptive discontinuation is one of the big problems of contraceptive methods use and an important issue in family planning service program. The discontinuation associated with method related reasons as side effects in hormonal methods, and non method related reasons as in marital separation. This discontinuation affect success rate of contraceptive method. Actually, side effect is the most common reason for discontinuation of the method. Most bothersome to women are headache, changes in menstrual bleeding (including irregular bleeding, spotting or the lack of menstrual bleeding), and uterine pain. Some women feeling worried about using the some method because the informations are not enough especially in rural areas. Urban women usually have better access than do rural women to contraception, to information about contraception, and to health care providers in case they have a problem using contraception. For these reasons, urban/rural residence is likely to affect both discontinuation and intention to use contraception. A woman’s level of education is likely to affect her knowledge of contraceptive methods and side effects, her preconceptions about family planning, and other factors may influence discontinuation or intention not to use contraception. Contraceptive use and method choice also vary widely by religion, parity and socioeconomic status. Reaching women who are poor and live in remote areas requires outreach by health personnel and community volunteers trained to provide integrated FP and MCH services. The discontinuation and non using of contraception can be eliminated by: • Counseling regarding safe and effective use of each method. GATHER approach help for googd counseling; Greet the clients respectfully, Ask them about their family planning needs, Tell them about different contraceptive options and methods, Help them to make decisions about choices of methods, Explain and demonstrate how to use the methods, Return/refer; when to return visit and follow up. • Earlier management of side effects as menstrual changes, headaches, dizziness, breast tenderness or bleeding. Explanation of these side effects and reassurance the client can be effective way to increase continuation rates. • Elimination of fears and misbelieves regarding religion and, contraceptive use for family planning. • Greater efforts need to encourage women to switch to modern methods rather than traditional methods, or even switch to another method that better than discontinuation of contraceptive use. • Efforts should be made to promote male involvement in the country’s reproductive health programs and encourage males to share in family planning; that has a good effect on continuation use of contraception and decrease the discontinuation. • Improvement of family planning services by continuous training of health team and increasing of community orientation about benefits of family planning and its effects on their life. |
