Abstract

Asthma is a serious global health problem affecting all age groups, with increasing prevalence in many developing countries, rising treatment costs, and a rising burden for patients and the community. The aim of the present study was to study Demographic and clinical characteristics of bronchial asthma patients and its relation to level of control in Al-mahalla chest hospital. This study was conducted at Al-Mahalla chest hospital, and included 200 adult patients with bronchial asthma having a wide range of asthma severity, from those attending the outpatient clinic and those admitted as inpatients.
Data from those patients were collected through:
Questionnaire, including:
Age.
Sex.
Occupation.
Smoking habit.
Age of onset.
Age of diagnosis.
Family history.
Level of asthma control.
Precepitating factors to asthma.
Other types of allergy
Inhalation therapy.
Education to use inhalation therapy.
Side effects of inhalation therapy.
Comorbidity presence.
Body mass index.
Peak flow meter.
In the current study the average age was 44.14 and average age of diagnosis was 29.23. As regard gender, 69.5% of the studied cases were females and mainly they were house wives. 43% of asthmatic patients were exposed to passive smoking and mainly they were females. In our study the average of smoking index of uncontrolled group was higher and the age of onset of smoking was earlier. Also percentage of current, passive and ex smokers was higher in uncontrolled group while controlled group has a higher percentage of never smokers.
Most of bronchial asthma patients were obese patients with average BMI 26.7kg/m2 and obesity was common among uncontrolled groups in comparison to controlled group. Asthmatic attacks were precepitated by infections in about 83.0% of the studied cases. 79.5% the studied cases suffer from attacks triggered by indoor allergens while outdoor allergens trigger attacks in 82.5% of cases. 46.0% of the studied cases had co-morbid conditions, allergic rhinitis was the commonest type of allergy72.1%.83.9% of the studied cases were treated by inhalation therapy. 46.5% of the studied cases had positive family history of the disease, 90.3 % of them from the first degree. 75.8% of controlled cases use inhalation therapy while 85.5% of uncontrolled cases use inhalation.
Complications from inhalation therapy was 7.8% among patients using inhalation therapy. As regard follow up of the patient in this study, it was found that 88% of the studied cases follow up with pulmonologist and 11.8% with internal medicine and 0.5% with general practitioners.
As regard severity we have no severe persistent cases in our study because of cooperation difficulities. 37.7% of our cases were intermittent, 24.6% were mild persistent, 37.7% were moderate persistent. Only 4.1 % of our patients have complications with asthma, most of them are uncontrolled cases. Examination of most of our cases show near normal vital signs except mild increase in respiratory rate.
