Abstract

Objective: The scientific discussion regarding the benefit of protection versus non protection in stenting of carotid artery (CAS) is still controversy since sufficient data comparing both treatment options are not available. To contribute further data to this important open question we conducted this comparative study of the clinical outcome following carotid stenting with or without the use of embolic protection device.
The material: of the present study consisted of 40 patients with symptomatic extra-cranial carotid stenosis more than 50% or asymptomatic stenosis more than 80% according to NASCET criteria, patients were randomized in Group A and Group B; each including twenty patients.
Group A: Included 22 stenotic lesions (20 symptomatic and 2 asymtomatic) in 20 patients, their mean age was 63.8 ±9.2 years, mean degree of stenosis was 78.18±10.01%, they undergone carotid stenting without the use of protection device.
Group B: Included 21 stenotic lesions (17 symptomatic and 4 asymtomatic) in 20 patients, their mean age was 65.1 ±8.45 years, mean degree of stenosis was 80.52±11.4%, they undergone carotid stenting with the use of protection device.
All patients were subjected to history taking, neurological deficit quantified according to the National Institutes of Health Stroke Scale (NIHSS), laboratory investigations, imaging and sonographic evaluation before the procedure and NIHSS and sonography during follow-up (6 months).
Results: primary stent placement was successful in all patients, the degree of stenosis decreased from a mean of 78.18±10.01% before the procedure to a mean of 11.36% ± 4.86 immediately after stent placement in group A and from a mean of 80.52±11.4% to a mean of 10.71% ± 6.57 in group B.
