Abstract

The aim of this study was to investigate the early results ”within three months” of limb revascularization using angioplasty of the peroneal artery as the only source for revascularization in treatment of critical lower limb ischemia.
In this current study we analyzed 20 limbs in 20 patients (55% males, the mean age was of 61.15±7.59 SD years) with critical lower limb ischemia (Rutherford categories 4 and 5), who underwent foot revascularization by endovascular balloon angioplasty. The end result after several trials to get through the anterior tibial artery or the posterior tibial artery or the peroneal artery or more than one tibial vessel was dilatation of the peroneal artery only. Additional interventions following the procedures were needed either minor amputations in 5 cases(25%), immediate debridement in 3 cases (15%) and minor amputation in one of them (5%) and in 2 cases (10%) minor amputations was done later on.
The main risk factors were as following: 9 patients were smokers (45%), 15 patients had diabetes (75%), 15 patients had hypertension (75%), 8 patients were cardiac (40%) and 3 patients were treated from renal impairment (15%).
The mean value of ABI before the intervention was 0.42±0.07 SD. During the follow up periods, the ABI values were recorded after one month and three months following the procedure with mean values as following: 0.68±0.07 SD, 0.68±0.07 SD. There were significant changes in the ABI values between the different follow up periods and before the intervention.
Complications from the intervention occurred only in 3 patients (15%). 2 patients of them (10%) had ecchymosis and small non communicating groin hematoma at puncture site. Another patient (5%) complained of communicating hematoma and pseudo aneurysm with ecchymosis and edema of the thigh.
The end results of the studied cases were as following: 10 of the cases (50%) were relieved from the rest pain, 6 cases (30%) had healed ulcers and another 4 cases (20%) had major amputations.
The limb salvage rate in our study was (80%) at 3 months. The wound healing rate at 3 months was (60%).Re intervention wasn’t done and it wasn’t needed.
