Abstract

The relationship between trauma, post-traumatic stress disorder (PTSD) and substance use disorder (SUD) has been long established in various studies and has been found that comorbid PTSD and SUD is associated with a more complex and costly clinical course when compared with either disorder alone, including increased chronic physical health problems, poorer social functioning, higher rates of suicide attempts, more legal problems.
The aim of the present work was to study the prevalence rate of trauma exposure and PTSD among inpatients with SUD in addiction management center at El-Maamoura psychiatric hospital.
In the current study a total of 200 inpatients with SUD were chosen from addiction management center in El-Maamoura psychiatric hospital.
For inclusion Patients were both males and females, aged from 18 to 65 years old. Participants were Patients with single or multiple substance use disorder admitted to addiction management center at El-Maamoura psychiatric hospital. Participants agreed to sign the consent.
Participants were assessed using a structured interview questionnaire to collect sociodemographic data, substance-use history, and medical and psychiatric history. The semi structured psychiatric interview was applied using DSM to assess severity of SUD and comorbid major depressive disorder (MDD), and psychometric assessment was performed using Arabic version of trauma history screen (THS) for assessment of trauma exposure ,and the Arabic version of PTSD Checklist, civilian version (PCL-C) for assessment of current PTSD.
The results were:
• Nearly all the sample (98.5%) were exposed to at least one type of trauma, with no statistical significance between males and females (98.7% and 97.7% respectively).
• There was high prevalence of PTSD in SUD inpatients (62.5%), and females showed higher prevalence of PTSD compared to males (77.3% vs.58.3%).
• The number of traumatic events experienced by the patients ranged from one to 13 events, the mean was 7.51 events. (SD±2.85). There were no difference between males and females regarding the number of traumatic events experienced by the patient (mean=7.52 ±2.76) vs (mean=7.48 ±3.20) respectively.
• The common trauma types were sudden death of a close family member or a friend (82.0%), sudden abandon by spouse, partner, parent, or family (75.0%). seeing someone die suddenly or get badly hurt or killed (72.5%), sudden move or loss of home and possessions (68.5%), attacked with a gun, knife, or weapon ( 65.5%).
• Women were more likely to experience sexual trauma, as a child, and as an adult, and some other sudden event that made them feel very scared, helpless
