Abstract

The current study was carried out to assess the prevalence of mental disorders among elderly admitted to the acute geriatric care and the impact on patient’s outcome.
All patients included in the study were admitted to the acute geriatric care settings in Ain shams University hospital, 54patients were admitted to the intensive geriatric care and 46 patients were admitted to the in-patient unit.
All were assessed using comprehensive geriatric assessment including: complete medical history taking, full examination with detailed neurological examination ,cognitive function assessment using AMTS, assessment for depression using GDS4 and CORNEL and function status assessment using ADL and IADL.
Subjects were assessed by the AMTS to assess cognition of the patients and CAM to assess delirium. Patients with intact cognition were assessed by the GDS4 and GAD7 to assess depression and generalized anxiety disorders but in those with cognitive impairment CAM was done for all of them to distinguish between delirium and dementia and all delirious patients were assessed for the severity of delirium by using DRS.
After resolving of delirium patients were assessed for depression using GDS4 and for generalized anxiety disorders using GAD7 for cognitively intact patients and for depression using CORNEL in demented patients.
APACHE II score was done for all patients admitted to the acute geriatric care as a predictor for the mortality index.
There is significant correlation between mortality and severity of acute illness on admission to the acute geriatric care using APACHEII score also, there is significant correlation between severity of delirium using DRS and mortality.
The current study showed that prevalence of cognitive impairment assessed by AMTS 45% of the subjects, delirium 40% and depression 67%, depression was more common in females than males, generalized anxiety disorders were in 3% and this was small percentage for further statistical analysis in our study.
In patients with cognitive impairment 51% died, in depression 13% of patients died and 55% of delirious patients died.
Prevalence of mental disorders was common among elderly admitted in ICU .Cognitive impairment is an independent predictor of mortality in our study.
