Abstract

This cross sectional study was done on 210 patients presented with chronic liver disease in Menouf Fever hospital. They were 130 males and 80 females. Their ages ranged from 40 years to 65 years. All patients were subjected to full history taking, full clinical examination, laboratory investigations, abdominal ultrasonography and upper GIT endoscopy.
The patients were divided into 2 groups:
 group I: include 140 patients with chronic liver disease and with esophageal varices and this group was subdivided into three subgroups according to the grade of esophageal varices:
- group I A: include 60 patients with esophageal varices grade I.
- group I B: include 40 patients with esophageal varices grade II.
- group I C: include 40 patients with esophageal varices grade III.
 group II: include 70 patients with chronic liver disease and without esophageal varices.
The aim of this study is to determine the correlation of the thrombocytopenia with esophageal variceal grading in chronic liver disease patients.
As regard the demographic data, there was no statistically significant difference between studied groups regarding age and sex.
On analyzing the clinical data, presence of palpable spleen by clinical examination was statistically significantly higher in patients with varices than without varices, and was higher in patients with grades II and III (large EV) than patients with grade I (small EV).
On analyzing the laboratory parameters, serum albumin and platelet count were statistically significantly lower in patients with
varices than without varices, and were lower in patients with grades II and III (large EV) than patients with grade I (small EV).
On the other hand, total bilirubin and PT (INR) were statistically significantly higher in patients with varices than without varices, and were higher in patients with grades II and III (large EV) than patients with grade I (small EV).
As regard the ultrasonographic findings, the portal vein diameter and spleen size were statistically significantly higher in patients with esophageal varices than without varices, and were higher in patients with grades II and III (large EV) than patients with grade I (small EV).
The results showed that there was a highly significant negative correlation between platelet count and esophageal variceal grading in chronic liver disease patients.
The results revealed that the platelet count at cut off value ≤ 130 x 103 can differentiate between patients with varices and patients without varices with sensitivity 95% and specificity 95%.
While at cut off value ≤ 80 x 103 it can differentiate between patients with esophageal varices grades II and III (large EV) and patients with grade I (small EV) with sensitivity 91.2% and specificity 86.7%.
