Abstract

This study was carried out at the Plastic Surgery Department, Sohag University Hospitalsin the period from January 2010 to December 2014.
The group of patients had varieties of soft tissue injuries of the foot. The ages ranged from 1.5 to 75 years. Various modalities of reconstruction were used. The total failure rate in this series was 6%. While the complication rate was 12%. Interesting finding in our study is the high incidence rate in children specially age group younger than ten years old in our community which need further investigations about the causes of such high incidence in this age group. Good Wound debridement is essential and should be accompanied with meticulous hemostasis. In our study, the main method in reconstruction of tissue defects are split thickness skin graft, fasciocutaneous flap, local skin flap and finally free flap. The ladder of reconstruction should be strictly followed and starting from the simple procedures and step by step climbing the ladder was our protocol in the management of soft tissue defects.
Although microsurgical progress has led to improvements in the quality of lower limb reconstruction, pedicled flaps remain good solution in selected cases. Recently fascial and fasciocutaneous flaps have become more popular and can provide an alternative for free tissue transfer.
Sever foot trauma whether resulting in primary amputation or foot salvage may cause a major alteration in life style in the form of chronic pain and ulcers, diminished function. These potential complications can be minimized by appropriate rapid management of soft tissue injuries.
Recommendations:
• Good wound debridement and meticulous hemostasis.
• There are no strict rules to follow in selecting the best way of reconstruction but with the good knowledge of the reconstruction ladder and its new modification besides considering the patients’ conditions and tolerability and the surgeon skill and experience will help him in the decision.
• The simple modality of reconstruction is the best.
More training in the microsurgical techniques and team sharing in foot reconstruction are highly recommended.
