Abstract

Acne vulgaris is a common human skin disease, characterized by areas of skin with seborrhea, comedones, papules , pustules , nodules and possibly scarring
(Adityan et al., 2009) .
Acne affects mostly skin with the densest population of sebaceous follicles; these areas include the face, the upper part of the chest,and the back(Harper,2009).
Acne scars are the result of inflammation within the dermis brought on by acne. The scar is created due to the healing process within the dermis leading to formation of excess collagen .Their are different types of acne scars including; ice pick scars, box car scars, rolling scars and/or hypertrophic scars (Goodman G ,2006).
Among the therapeutic tools for treatment of acne scarring are ; excision and subcision , punch techniques, dermal grafting, needling and combined therapies for atrophic scars as silicone gels, intralesional steroid therapy, cryotherapy, and surgery for hypertrophic and keloidal lesions ( English and Shenefelt ,1999) Skin resurfacing options including ;laser, dermabrasion , medium –depth to deep chemical peeling are options of the treatment tools( Jacob et al.,2001).
Chemical reconstruction of skin scars (CROSS) is a technique using high concentrations of trichloroacetic acid (TCA) focally on acne scars to induce inflammation followed by collagenisation. This can lead to reduction in the appearance of scars and cosmetic improvement (Lee et al.,2002).
Fillers are another option in the treatment lines of atrophic post acne scarring , dermal filler is a product that is injected or placed into the dermis leading to correction of individual lines and depressions (Alam et al.,2008) Injectable fillers can enable short-, medium-, or long-term correction of acne scars. Large-particle fillers such as calcium hydroxylapatite have a longer persistence in vivo and are appropriate for larger areas of rolling scars; more viscus hyaluronic acid can be injected directly beneath individual pitted or box-car scars.Volumetric fillers, such as poly-L-lactic acid,is controversial(Barnett and Barnet,2005).
Our study proved that hyaluronic acid filler injection is a good option in treatment of atrophic acne scars as there was a highly significant difference between amount of collagen deposition before and after treatment with (P<0.001)), mean collagen of 49.30 before and 55.02 after treatment also improvement in the quality and remodeling of collagen bundles and fibroblast in papillary dermis was recorde in our study .
Clinical improvement in scar scoring was recorded in 100% of patients with highly significant value (P <0.0001) ) as 30% of the cases turned from mild to macular ,60% improved from moderate to mild ,10% improved from severe to moderate grade and no patients were presented with severe grade.
