Abstract

Ovarian cancer is the second most common type of female reproductive cancer, despite that more women die from ovarian cancer than from cervical cancer and uterine cancer combined.
Screening for any type of cancer must be accurate and reliable; it needs to accurately detect the disease and it must not give false positive results in people who don’t have cancer.
There has been significant effort in the research and development of novel biomarkers applicable to the diagnosis and management of epithelial ovarian cancer. One of the emerging biomarkers in addition to serum CA125 in this field is the hepatocyte growth factor.
Hepatocyte growth factor (HGF) is a multifunctional growth factor, mainly produced by mesenchymal-derived cells. Within the ovary, HGF controls numerous key functions which collectively regulate the growth and differentiation of ovarian follicles.
The aim of the present study was to evaluate as a screening marker the serum level of hepatocyte growth factor among suspicious malignant adnexal masses as compared to serum levels of CA125.
The study was conducted on 80 cases (40 cases with suspicious malignant adenxal masses and 40 cases with benign gynecological conditions as a control group).
Clinical examination, imaging studies, as well as laboratory determination of selected hematological and biochemical parameters including determination of serum HGF level were done to all subjects. Optimum surgical staging was done for cases with suspicious malignant adenxal masses, with histopathological examination of surgically resected ovarian masses or biopsies obtained from ovaries.
Except for the statistically significant differences noted in age and BMI between the studied groups, no significant differences were noted in the menstrual status (pre versus post menopausal), parity (nulliparous versus multiparous), hematological and biochemical parameters done in this study.
Regarding the histopathological findings, 12.5% cases had borderline tumor and 87.5% had malignant tumor, with serous cystadenocarnioma (42.5%) as the most frequent type followed by mucinous cystadenocarnioma (35%) and lastly the endometriod adenocarninoma of ovary (10%).
As regards tumor markers done in this study, serum CA125 showed a significantly lower mean value in cases with borderline tumor compared to those with serous tumor In addition, its serum level was higher in cases with pelvic LN involvement.
As regards serum HGF, a significantly higher mean value was observed in cases compared to control group. Furthermore, the relation between serum hepatocyte growth factor and various histopathological findings was evident in this study, with highest level observed in serous tumor followed by mucinous tumor and lastly endometroid tumor.
