Abstract

Chronic kidney disease (CKD) is a growing public health problem worldwide with treatment options of either life-long hemodialysis or renal transplant. The maintenance hemodialysis patients usually experience high levels of psychological stress result in psychological distress symptoms, such as depression and anxiety. Unfortunately, health care providers remain largely unaware of both the presence and severity of psychological disturbance among hemodialysis patients, especially for healthcare providers who were not training in psychiatric evaluation. This study aimed to assess the impact of applying a ’’Cognitive Behavioral Therapy program ’’ as a psychiatric nursing intervention on depression and anxiety in patients on maintenance hemodialysis and on their coping strategies. A quasi-experimental research design [pretest -posttest] was conducted at the dialysis units of the New Mansoura General Hospital. The instruments used for data collection were Socio-demographic and Clinical Data Structured Interview Schedule, Beck Depression Inventory-II, Beck Anxiety Inventory, and Dealing with Illness Inventory. Two hundred thirty eight patients undergoing hemodialysis were screened for inclusion and exclusion criteria and 44 patients were selected randomly through Simple Random Sampling Method and recruited for the study. These patients divided into four groups, two male groups and two female groups, whereas 41 patients of all groups completed the study after applying Cognitive Behavioral Nursing Intervention Program with 12 weekly sessions. The study results reveal that there was statistical significant difference in scores of depression, anxiety, and coping strategies between pre and post the intervention program. Application of psychiatric intervention program especially CBT in hemodialysis units is recommended.
