Abstract

Childhood is a critical stage of development that forms the foundation of the person future wellbeing and learning. Nowadays the number of children with severe and complex behavior disturbances is increasing. The use of psychotropic drugs in children has increased significantly in the past two decades, However psychotropic medications use other than stimulants have not been adequately studied in children in terms of benefit and safety. The aim of our study is to assess the degree of concordance between prescription pattern for child psychiatric disorders in primary health care facilities in Egypt and the international treatment guidelines. The current study is a cross sectional study that is conducted to 272child attended to Mansoura university hospital child psychiatry outpatient clinic. Our study include both sexes, Children aged two to twelve years old, who are visiting MUH child psychiatric clinic for the first time and children with previous psychotropic drug prescriptions. The results of our study show that, ADHD and disruptive behavior disorder (ODD and conduct disorder) represent most of the diagnoses of the studied group, regardless the specialty of the prescriber. As regard agreement of provisional diagnosis versus MINIKID, one third of the diagnosis of the studied group was agreed with psychiatrists has the highest diagnostic accuracy. antipsychotics and stimulants drug prescription is significantly higher than other psychotropic drug prescription. Among the factors that may contribute in the disagreement with the Maudsely guidelines, absence of psychotherapeutic intervention is considered the most frequently seen factor. The cases with no improvement, their pattern of psychotropic prescription was not agreed with Maudsely guidelines. On the other hand near half of the cases with near totally improvement, their pattern of psychotropic prescription was agreed with Maudsely guidlines. About half of the cases received behavioral therapy, more than half of them show near total improvement. It can be concluded that: The degree of concordance between psychotropic medications prescribed for children referred to child psychiatry clinic, Mansoura University Hospitals and the Maudsely guidelines was ordered as follow: 42.3% disagreed, 34.2% partially agreed and 23.5 totally agreed, with psychiatrists have the highest degree of ”totally agreed” among the prescribers and the neurologists have the highest degree of ”disagreed” among the prescribers. Antipsychotics and ADHD medications (Atomoxetine and stimulants) prescriptions are significantly higher than other psychotropic drug prescription. Absence of psychotherapeutic interventions is considered the most frequently seen factor among factors leading to disagreement with the Maudsely guidelines. The more the agreement of the prescriptions with the Maudsely guidelines, the more the improvement and the less the side effects.
