Abstract

Palmar psoriasis, hand eczema and tinea manuum assume considerable importance as clinical confusion in their diagnosismight occur. Separation of each of these is important because the treatment, life style,percussionsandprognosis of each disease is specific. Unusual presentations of kerato derma diseases do exist at times and that may cause difficulties in the differentiationamong these entities. A dermoscope (dermatoscopy) is a non-invasive, diagnostic tool whichvisualizes subtle clinical patterns of skin lesions and subsurface skin structures notnormally visible to the unaided eye. Hence, this office procedure may obviate the need for a skinbiopsy for diagnosis and for follow-up. The facility of storage of images and theresults being immediately available are added advantages The aim of this work was to study the dermoscopic features of palmar psoriasis, hand eczema and tinea manuum compared with each other. The study was performed on 60 patients among them palmar psoriasis, hand eczema and tinea manuum. The studied patients were subjected to complete history taking, general and dermatological examination, clinical digital photographs by Sony Steady Shot DSC-W800 20.1-Megapixel Digital Camera, 5X Zoom Lens. Dermoscopic examination was done by Dermlite II PRO HR (3 Gen, USA).Alcoholic solution was used to decrease density of scales that obscure vascularity appearance.The results of this study revealed the following: I-Palmar psoriasis patients: Palmar psoriasis patients number were 28, including 22 males and 6 females. Average age, 50.50 years [range, 34–65]. On dermoscopic examination of the included Ps patients, diffuse thick white scales and light red erythema were present in all examined lesions (100%), dotted blood vessels were present in 17 lesions (60.7 %) which were diffuse in 15 lesions(53.6%) and patchy in 2 lesions (7.1%).Yellow vesicles were detected in (17.9%). II-Hand eczema patients: Hand eczema patients number were 27, including 9 males and 18 females. average age was 32 years[range, 11–54] On dermoscopic examination of the included hand eczema patients, sparse white scales were present in 25 lesions (92.6%), yellow scales in 19 lesions (70.4 %) , orange globules were present in 23 lesions (85.2%) , patchy dotted blood vessels were present in 21 patients (77.8 %) , serocrust were present in 20 lesions (74.1% ) on dull red erythema in 15 patients (55.6%) or orange erythema in 12 patients (44.4%). III-Tinea manum patients Tinea manum patients’ number was 5, including 1 male and 4female.average age was 29 years[range, 27–57] On dermoscopic examination of the included tineamanumpatients, white scales mainly localised in the creaseson top of orange erythema were present in all examined lesions (100%).Patchy dotted blood vessels were found in 1 patient (20%).
