Abstract

Proper treatment of Perianal fistula should consider the following criteria to help complete eradication of fistula and maintenance of continence and prevent recurrence depending upon accurate pre-operative assessment of fistula geographically, namely the site and level of any internal opening, the anatomy of primary track, and the presence of any secondary ramifications, EAUS plays a major role in the preoperative assessment of perianal fistulas.
The current study included 50 patients (32 males and 18 females; age range 10-60 years; mean age 34 years) with clinical manifestations of perianal sepsis , examination was done for all patients in ultrasound unit, Radiodiagnosis and Imaging Department, Zifta general Hospital, using GE P5 logiq ultrasound equipment with 3 dimensional (3D) capabilities.
The patients studied consist of 8 patients with Grade I 16 % , 6 with Grade II 12 % , 18 with Grade III 36 % , 16 with Grade IV 32% , 2 with Grade V 4% , in this study 50 patient underwent surgery, 6 patients showed Grade I simple linear intersphincteric fistula 12 % , 8 patients showed Grade II intersphincteric fistula with secondary tracts and intersphincteric abscess 16 % , 14 patients showed Grade III simple trans-sphincteric fistula 28 % , 18 patients showed Grade IV trans-sphincteric fistula with abscess and secondary tracks 36 % , 4 patients with Grade V supralevator and translevator disease 8% , the EAUS accuracy for depiction of both primary tracts (sensitivity 95 %; specificity 86 % ; accuracy 80 %) and abscesses (sensitivity 100% ; specificity 100% , accuracy 100 % ) and internal opening (sensitivity 100% ; specificity 97
