Abstract

Competency based medical education is being increasingly applied especially in graduate medical education. It requires greater emphasis on the assessment to evaluate the competencies and hence a special care for the workplace-based assessment (WPBA). The Multisource Feedback is increasingly applied in the postgraduate studies as a workplace-based assessment tool that relies on observation of the residents’ behavior by his peers. The Sheffield Peer Review Assessment Tool (SPRAT) is the original questionnaire-based tool of multisource feedback that was validated to assess UK pediatric residents. The SPRAT is composed of 24 questions that cover the 5 domains of Good Medical Practice 2006 proposed in UK, the 5 domains are: “Good clinical care”, “Maintaining good medical practice”, “Teaching and training, appraising and assessing”, “Relationship with patients” and finally “Working with colleagues”.
Aim of the work:
This study aimed to apply the SPRAT as a WPBA tool for the pediatric residents at El- Shatby University Hospital (ESUH), to explore its psychometric characters including selected types of validity, reliability, acceptability and feasibility and to investigate the factors that can affect the residents’ score including the duration of his training at ESUH; having qualifications other than the pediatric training e.g. attending workshops, courses, conferences, and continuing medical education; his graduation grade and the length of the working relationship with his assessors.
Subject and methods:
The study was a psychometric cross-sectional survey. Both qualitative and quantitative approached were used. The pediatric residents and visiting trainee at ESUH during the period of October 2016 to January 2017 enrolled in the study (n= 64) provided that they had no less than two months of pediatric training and no more than five years. They filled a self-administered questionnaire requesting the following data: their name, graduation year, their duration of training, previous qualifications, graduation grade and the names of 10 potential assessors that should include 5 supervisors and 5 peers or subordinates with the duration of their working together. Filling of this questionnaire was considered the resident’s consent for enrollment in the study. Each resident also filled a SPRAT form as a self-assessment. The nominated assessors (n= 125) were contacted to complete SPRAT forms for their corresponding residents. The data obtained from the questionnaire was kept confidential to ensure just and honest evaluations.
The following data was calculated from the collected assessments:
o The mean performance of each resident per question
o The performance of all the included residents per question
o The mean and median domain scores of each resident
o The mean and median domain scores of all included resident.
