Abstract

Introduction: Cesarean delivery is the most common and major obstetric operative procedure worldwide and cesarean rate has been continuously increased. Cesarean section is defined as the surgical termination of pregnancy or delivery by operative opening of uterus.
Aim of the Work: The aim of the current work is to compare two different techniques of delivery of the fetal head at time of cesarean section. Manual and forceps delivery of the fetal head as regards the incidence of unintended uterine extension, hemoglobin change 24 hours after cesarean section.
Patients and Methods: The present study is a RCT (randomized control trial) was conducted at Ain Shams University Maternity Hospital and was done to compare two different techniques of delivery of the fetal head at time of cesarean section. Manual and forceps delivery of the fetal head as regards the incidence of unintended uterine extension, hemoglobin change 24 hours after cesarean section.
Results: The mean age among study group 1 cases was 27.2±3.4 with a range of 21-36 year. The majority of cases were P2 (75.3%). More than half of cases (52%) had no previous abortions. While, the mean age among study group 2 cases was 27.7±4.8 with a range of 21-39 year. More than half of cases (54%) were P2 (54%). Almost half of cases (49.3 %) had no previous abortions.
Conclusion: Thus from the present study we can concluded that Forceps is better than and has less complications than manual delivery of the fetal head at time of elective cesarean section as regards unintended extension, hemoglobin change 24 hours after cesarean section.
Recommendations: Forceps extraction of fetal head during cesarean section is superior to manual delivery in cases of difficult extraction such as floating head.
